A6.3 City Lab

This sub-project (demonstration in a local sample health system - CITY LAB) has the following goals:

• Survey of existing health-promoting activity models based on a sample of local authorities nationwide
• Analysis of the strengths and weaknesses of existing models and the main opportunities for improvement found;
• Analysis and proposal of protocols for coordination between community health promotion structures and health, education and social security institutions;
• Development of models for remote monitoring of parameters and self-care;
• Development of clinical governance models for the transfer of test results, remote consultations, drug delivery, access to care, monitoring;
• Case-control study of the proposed activities (intervention in half the city while keeping the other half as a standard; Analysis and proposal of a patient-centered care model).
